EXPRESSION OF PROLIFERATING CELL NUCLEAR ANTIGEN IN PLEOMORPHIC ADENOMAS ON THE BASIS OF HISTOLOGIC FINDINGS

Abstract

The pleomorphic adenoma, the most common tumor of the salivary gland, is
characterized by a variety of histologic findings, such as myxochondroid area, stromal
hyalinization, and squamous metaplasia. In particular, the infiltrative growth, the nuclear
pleomorphism, vasecular encroachment and focal necrosis are not infrequently found in
benign pleomorphic adenoma. But the biologic behavior of these findings has to be
elucidated.
In an attempt to evaluate the prognosis of pleomorphic adenoma on the basis of the
variable histologic findings, proliferating activity was analysed by use of
immunohistochemical study for proliferating cell nuclear antigen(PCNA).
One hundred seventy three cases of benign pleomorphic adenomas and 7cases of
malignant mixed tumors were examined by clinically and histopathologically. The
anti-PCNA antibody(PC 10, DAKO) was applied. Results were as follows :
1. Thirty six percents of benign pleomorphic adenomas showed squamous metaplasia,
and the predominent myxochondroid area was seen in 28.3£¥ of the cases. The area of
capsular invasion and vascular encroachment were shown in 59.0£¥ and 63.0£¥,
respectively.
2. Cases with the area of focal necrosis and/or nuclear pleomorphism revealed high
PCNA indices.
3. The multinodular growth was th most common finding in recurrent cases, however,
the difference of PCNA reactivity between recurrent cases and non-recurrent cases was
not found.
4. Malignant mixed tumors showed the highest PCNA index particularly in
carcinosarcoma.
In conclusion, immunohistochemical stain using PCNA could be a useful tool to
distinguish the benign from malignant mixed tumors. Although the cases showing
nuclear plemorphism and/or focal necrosis revealed high PCNA reactivity, it is necessary
to investigate their clinical significance coupled with long term clinical follow-up.
